 How to complete the CAPS Transfer and Authority to Direct Payment form

 

The CAPS Transfer and Authority to Direct Payment form can be used to authorise Medicare Australia to deposit the CAPS payment for yourself or your client directly into an account held at Intouch rather than your own bank account.

 

Benefits of managing your CAPS in this manner include:

 

  • one single point of contact for accessing CAPS funding and ordering products

  • continuation of product purchase through Intouch like you did under CAAS with Intouch managing your funds on your behalf.

  • confidence that the CAPS payment will be kept separate from your everyday funds and therefore available for purchasing your continence aids when you need it.

 

This guide will assist you to complete the CAPS Transfer and Authority to Direct Payment form in order to authorise Medicare to deposit the CAPS payment directly with Intouch for management of funds on your behalf. 

 

  1. Is the CAPS recipient an existing member of the CAAS AND have they been a member of the   CAAS since before 1 April 2010?

           

Yes

 Please complete the CAPS Transfer and Authority to Direct Payment form using the guide below.

No

 If you are not a current CAAS client, please contact Intouch before June 30 to discuss eligibility and application process.

 If you are a current CAAS client who has joined the scheme recently (i.e. since 1 April 2010), you will be sent a CAPS transfer form during July/August and cannot use this form for direct payment until your invitation to transfer is received.

 

  1. Are you completing the form yourself for your own funds or on behalf of someone else receiving the CAPS payment?

  1. If you are the CAPS recipient and are completing the CAPS Transfer and Authority to Direct Payment for yourself with no other correspondence or legal representative required to authorise payment of your CAPS funds directly to an account in your name at Intouch,  please follow our step by step guide here.

If you are completing the form on behalf of a CAPS recipient whereby you are the client’s representative and are signing the form on behalf of the client AND are authorising Medicare to deposit the CAPS payment directly into an account at Intouch,  please follow our step by step guide here.

Intouch will check your form, sign the organisation acceptance section (Q35-43)

and forward to Medicare on your behalf.
