CH2 Aged & Community Care is a national supplier of continence products for the Department of Veterans' Affairs Rehabilitation Appliances Program (DVA RAP). DVA RAP is an Australian Government program which provides aids and appliances to eligible members of the veteran community to help them maintain their independence.
Eligibility
If you have a Gold Repatriation Health Card for All Conditions you may be able to obtain aids and appliances when you have an assessed clinical need.
If you have a White Repatriation Health Card for Specific Conditions you may be able to obtain aids and appliances for an accepted disability when you have an assessed clinical need.
Note: As of 1 July 2007, veterans from allied countries are eligible for RAP equipment or home modifications provided by DVA in relation to accepted disabilities.
Application and ordering
To enable CH2 Aged & Community Care to process an order, we must receive a copy of the entitled person's Direct Order Form for continence products. This is available for download from our forms page, and is accompanied by a fax cover sheet you can use to return the form to us.
The Direct Order Form must be filled out by an approved prescriber, including continence nurse advisors (registered nurses or physiotherapists), registered nurses (Div. 1) and urologists. Occupational therapists are able to prescribe certain items. Local medical officers may also prescribe, although DVA's preference is for them to refer to continence nurses/clinics.
The Department of Veterans' Affairs RAP Product Catalogue can be downloaded from our forms page and outlines the specific products available for prescription under the DVA RAP. There are also prescriber guidelines available for download for your information.
Once we have received the Direct Order Form, we will process the order and forward it to the entitled person within two working days for metropolitan areas and between three to five working days for regional to remote areas (dependent on stock availability).
A prescriber or entitled person can then contact us at any time to re-order items listed on the Direct Order Form, up to the prescribed allowable limit. Items not listed on the form will need to be approved by the DVA prior to CH2 Aged & Community Care processing the order.
Please note that an approved Direct Order Form will, upon processing, be automatically fulfilled to the client up to the prescribed allowable limit, and will be valid for quarterly re-order for up to two years provided the product does not need to be changed.
Translation service
For people from non-English speaking backgrounds, we offer a translation service at no extra cost. To ensure accurate consultation to all our customers we have access to over 1 300 interpreters speaking more than 120 languages and dialects. Simply advise your CH2 Aged & Community Care Healthcare Consultant of the language required to access this service.
